Machine-related incidents usually trace back to a guard that was removed, a sensor that was bypassed, or maintenance that was deferred.
Check on the machine
- All fixed guards in place, secured, and not modified
- Interlocked guards actually stop the machine when opened
- Light curtains and safety mats tested and functional
- Emergency stops accessible and tested within the last month
- No safety devices bypassed, taped, or jumpered
- Pinch points and rotating parts covered
- Machine stops within the expected time after E-stop
Check on condition
- No abnormal vibration, noise or heat during operation
- Lubrication points serviced within the schedule
- No visible oil or coolant leaks creating slip hazards
- Guarding on belts, chains and couplings intact
- Preventive maintenance completed on schedule, not deferred

2. Electrical and control systems
Electrical faults give warning. A hot terminal, a smell of burning insulation, a breaker that trips more often than it used to.
Check on panels and distribution
- Panel doors closed and secured, not propped open
- No exposed live parts or missing blanking plates
- Earthing continuity checked and recorded
- Thermal imaging carried out on critical panels
- Cable glands intact, no cables through unprotected holes
- Panels not used for storage
- Circuit labelling accurate and legible
Check on control and automation
- Safety interlocks tested, results recorded
- Robot cell fencing, gates and interlocks functional
- PLC safety functions not overridden in software

Software overrides are the item most often missed. A safety interlock disabled in the PLC to keep production running does not show up on a physical inspection.
3. Maintenance practice
Plants running mostly on breakdown maintenance carry higher safety risk. Work is done under pressure, often outside normal hours, sometimes by whoever is available rather than whoever is qualified.
Check on the maintenance system
- PM compliance rate for the last three months
- Number of deferred PM tasks and reasons
- Breakdown frequency trend, rising or falling
- Repeat failures on the same equipment
- Critical spares available on site for critical machines
- Work permits used for high-risk maintenance tasks
- Root cause analysis completed on major failures
PM compliance below 80 percent is a safety finding, not just a maintenance one. It means scheduled checks are being skipped on equipment people work around every day.

4. Workforce competency and training
A well-guarded machine operated by someone who has not been trained on it is still unsafe.

- Every operator trained on the specific machine they run
- Training records current and signed
- Refresher training completed on schedule
- Competency verified, not just attendance recorded
- Trained cover available for critical roles across all shifts
Check on the shop floor
- PPE worn correctly, not just issued
- Correct PPE for the task, not generic issue
- Contract workers completed site induction before starting
- Induction records available for every contractor currently on site
- Toolbox talks held and recorded
The contractor induction item is where most plants have a genuine gap. Induction gets compressed when a contractor is needed urgently. Ask for records of everyone on site today and see what comes back.
5. Staffing and shift cover
Safety depends on having the right people available at the right time. This is the checklist most audits skip entirely.
Check on coverage
- Qualified electrical person available on every shift
- Trained first aiders present across all shifts
- Emergency response team members identified and available
- Night shift maintenance cover adequate for critical equipment
- Single points of dependency identified where only one person can do a task
- Contractor supervision present, not just contractor workers
If a critical repair can only be carried out by one named individual, that is a finding. Plants often close this gap with deployed technical manpower rather than carrying additional permanent headcount.
6. Shutdowns and high-risk work
Shutdowns concentrate risk. Unfamiliar contractors, unfamiliar tasks, several teams working in the same area, and a schedule that everyone is trying to beat.
Check on isolation
- Lockout tagout applied and verified, not assumed
- Stored energy released before work starts
- Each worker applies their own lock where required
- Isolation verified by test, not by looking at the switch
Check on high-risk activity
- Confined space entry permit issued, atmosphere tested, standby person present
- Hot work permit issued, fire watch in place, area cleared
- Working at height controls in place
- Lifting equipment inspection certificates current
- Rigging plan reviewed for heavy lifts
- Multiple contractors coordinated so work does not conflict

- Closure verified by someone other than the person who did the work
- Open findings from previous audits reviewed
- Repeat findings flagged and root cause examined
